International Health Insurance in China: What to Check Before Care
A policy-first checklist for international health insurance in China: benefits, exclusions, networks, direct billing, pre-authorization, reimbursement, and documents to confirm.

International health insurance does not answer βAm I covered in China?β with one yes or no. Coverage depends on the exact policy, benefit category, area of cover, hospital and campus, department, treatment, medical-necessity rules, exclusions, benefit limits, deductibles, co-payments, and pre-authorization requirements.
A hospital may offer direct billing for some plans while requiring other patients to pay first and claim reimbursement. Even when direct billing exists, uncovered charges remain the patient's responsibility. Before care, ask the insurer to confirm the named provider and service in writing, and ask the hospital whether it can use that billing route for your plan.
This guide helps you identify the questions. It does not interpret a policy, verify eligibility, or estimate reimbursement.
Scope: International Health Insurance, Not Travel Insurance
This page focuses on international or expatriate health insurance used for ongoing healthcare across countries. It is not a guide to standard travel insurance.
Travel insurance is generally designed around risks during a trip. UK government guidance tells travelers to check emergency treatment, hospital type, transport, repatriation, trip length, and pre-existing-condition terms. Separate government guidance warns that standard travel insurance does not normally cover travel undertaken for elective surgery.
If planned medical care is a purpose of the trip, disclose that purpose and request a written answer. Do not assume that a policy sold for travel and a policy sold for international healthcare have the same scope.
Build a Policy Coverage Map
Do not begin with a hospital logo list. Begin with the policy documents available in the insurer's member portal: the certificate or schedule, table of benefits, benefit guide, exclusions, endorsements, and claim instructions.

The presence of a category in a policy does not prove that every service within it is covered. Use the exact treatment and provider when requesting confirmation.
Network Status Is More Specific Than a Hospital Name
Ask the insurer to verify:
- the hospital's legal name
- the campus or location
- the regular, VIP, private, or international department
- outpatient, day-care, or inpatient status
- the named doctor or provider type, if relevant
- the planned service or treatment category
- the intended date or authorization validity period
A large hospital can operate different campuses and billing routes. A general directory entry may not apply to every department or service.
The Shanghai government profile for Huashan Hospital publishes organizations associated with direct-pay services. Renji Hospital also publishes a direct-payment page and says its insurer directory is frequently updated. These pages show that a billing mechanism exists; they do not confirm a specific patient's eligibility.
Direct Billing vs Reimbursement
Direct billing
With direct billing, the hospital submits eligible charges through an insurer or assistance company. Before the visit, confirm:
- Whether the exact plan can be used at the hospital, campus, and department.
- Whether a guarantee letter or pre-authorization is required.
- Which charges must still be paid at registration, discharge, or the pharmacy.
- How deductibles, co-payments, excluded items, deposits, and benefit limits are settled.
- What happens if authorization has not arrived when care is due.
Renji's published workflow explicitly warns that direct billing does not ensure complete coverage and that the patient must pay for items outside the policy.
Pay first and claim reimbursement
When reimbursement applies, collect the official receipt, itemized bill, medical record or diagnosis evidence, proof of payment, and any additional document required by the plan. A public international-insurer FAQ states that a tax invoice is required for claims involving treatment in China under its standard process. That is a plan-specific example, not proof that every insurer uses the same rule.

Pre-Authorization and Guarantee Letters
These terms can describe different functions:
- Pre-authorization is the insurer's advance review under policy rules.
- Guarantee of payment or guarantee letter is a payment arrangement sent to the provider, often for direct settlement.
- Benefit verification confirms policy information but may not guarantee final payment.
Ask whether approval covers the exact hospital, dates, service, and estimated amount, and whether any condition remains. Save the authorization number and written approval. If the proposed care changes, ask whether a revised authorization is required.
Renji's International Medical Service tells commercial-insurance patients to notify its insurance specialist when hospitalization is proposed so the hospital can assist with an inpatient guarantee-letter application. That workflow is specific to the hospital and insurer involved.
Terms That Can Change What the Insurer Pays
Locate these in your own policy rather than relying on a summary:
- deductible or excess
- co-payment or co-insurance
- annual, lifetime, or benefit-specific limit
- waiting period
- pre-existing-condition exclusion, moratorium, or endorsement
- medical-necessity or reasonable-and-customary-charge rule
- provider, network, country, or area-of-cover restriction
- pre-authorization requirement
- excluded treatment, condition, medicine, device, or service
- claim deadline and evidence requirements
Do not ask only βIs it covered?β Ask the insurer to identify the benefit section, exclusions, cost sharing, authorization, limit, and documents that apply.
Twelve Questions to Send Before Care
I am considering care at [hospital legal name], [campus], [department], in China. Please answer the following in writing for policy [number] and the proposed [consultation/treatment].
- Is mainland China within my area of cover for this service?
- Is the hospital, campus, department, and provider eligible?
- Which benefit category applies?
- Is the care subject to an exclusion, waiting period, or pre-existing-condition term?
- Is pre-authorization required, and what information must be submitted?
- Is direct billing available for outpatient care, inpatient care, or both?
- Is a guarantee letter required, who requests it, and how long is it valid?
- What deductible, co-payment, co-insurance, sub-limit, or benefit limit applies?
- Which charges must I pay directly even if direct billing is arranged?
- If I self-pay, which receipt, itemization, medical record, claim form, proof of payment, and translation are required?
- What is the claim deadline and accepted submission channel?
- Are follow-up care, complications, changed travel, or treatment after returning home addressed separately?
Save the answer, policy documents, authorization reference, and contact details.
What to Ask the Hospital
- Does the named department use direct billing for my exact plan?
- Which insurer or assistance contact should send authorization?
- Can the hospital provide a written first-step estimate?
- What deposit or payment is required before care?
- Which official receipt and itemized fee documents will be issued?
- Can the hospital complete the insurer's form, and when?
- Are English records available, or must I arrange translation?
- Who handles billing questions after discharge or departure?
